About this clinic

The Health Care for the Homeless (HCH) Program of Manchester at Catholic Medical Center, is made possible through generous support from the Manchester Health Department and Catholic Medical Center. HCH Manchester conducts a primary care clinic at two locations: one clinic daily at the New Horizons for New Hampshire Shelter, and another clinic twice weekly (Monday mornings and Thursday afternoons) at the Families in Transition housing program. All persons who are experiencing homelessness, or who are at risk of being homeless, are welcomed. No one is turned away due to an inability to pay.

What should I bring to a first visit?
Usually a photo ID, something showing your address, proof of income or a self-declaration, your insurance or Medicaid card if you have one, and a written list of your current medications and doses. Call ahead — requirements vary, and arriving without the right paperwork is the most common reason a first visit gets rescheduled.
Do I need insurance, or immigration documents?
Federally funded health centers serve patients regardless of insurance status and do not require proof of citizenship to provide care. Volunteer and charitable clinics set their own rules. If it matters to you, ask before your visit what they require and what they record.
Is dental care included?
Usually not, and this is the single most common disappointment. Dental is frequently a separate program with its own eligibility, its own waiting list, and a narrower service list — extractions and urgent pain may be covered while fillings, crowns, root canals, and dentures are not. Confirm that dental is offered at this specific location before travelling.
Can I get a prescription refilled here?
Most clinics require an appointment before they will refill anything, and a first appointment can be weeks out. If you are close to running out of a medication you cannot safely stop, say so when you call — say the medication name and how many days you have left. Ask your pharmacy at the same time; pharmacists can sometimes authorise a short emergency supply.
What if I need care today?
