Details: Sweet Beginnings Womens Resource Center is committed to providing quality support services to women in our community who are experiencing an unplanned pregnancy or need assistance in parenting children from infancy to age 3. We are committed to ending infant abandonment and child abuse by providing education, support, resources, housing resources, referrals to medical care and adoption services when needed.
Details: We believe in caring for the whole person. The Willow Pass Wellness Center was conceived as a collaborative effort to integrate behavioral health and primary care for county mental health patients. We serve central county residents being seen at any Contra Costa Health Services adult mental health clinic, but primarily work with patients from the Central Adult Mental Health Clinic, which is located in the same building as the Wellness Center. We are a team of physicians, primary care providers, nurses, mental health case managers, community support workers, and administrative staff, all dedicated to providing the best possible care for our patients. We seek to improve access to primary care for patients with mental health needs by having primary care services located in the same place and by assisting with transportation. Our collaborative efforts work to provide the best care for patients.
Details: Since 2003, La Clínica Monument has provided the Central Contra Costa community with affordable, culturally sensitive, health care services. To meet the growing demand in that region, we moved to a beautiful, new building in Concord that will allow us to serve more than 15,000 patients annually by 2016. The new facility provides expanded medical care, dental care and optometry services, as well as health education, chronic disease management, senior services, OB/GYN and pediatric care. One of our most modern health centers, it is our first solar energy building!
