As a Certified Primary Care Medical Home, we focus on building a strong relationship with you to understand your unique needs, history, and preferences. This helps us create a treatment plan that’s just right for you. Our team works together to provide care for everything, from regular check-ups to serious health problems, including mental health. By getting to know you well, we make sure you get the best care and feel comfortable talking to us. This way, we help you stay healthy and happy. Horizon is a non-profit integrated health care provider licensed in Arizona and accredited through the Joint Commission to deliver a wide variety of inpatient, outpatient and residential services for all ages and stages of life. Is this clinic actually free?

What should I bring to a first visit?

Do I need insurance, or immigration documents?

Is dental care included?
Usually not, and this is the single most common disappointment. Dental is frequently a separate program with its own eligibility, its own waiting list, and a narrower service list — extractions and urgent pain may be covered while fillings, crowns, root canals, and dentures are not. Confirm that dental is offered at this specific location before travelling.
Can I get a prescription refilled here?
Most clinics require an appointment before they will refill anything, and a first appointment can be weeks out. If you are close to running out of a medication you cannot safely stop, say so when you call — say the medication name and how many days you have left. Ask your pharmacy at the same time; pharmacists can sometimes authorise a short emergency supply.
What if I need care today?
