Clinic Details:HealthCare Choices (HCC) is a not-for-profit organization that has been a NYS licensed diagnostic and treatment center since 2001 and a Federally Qualified Health Center since 2011. It has been a leader in offering integrated primary and behavioral health services as well as dental and other specialty care to the community, including special populations. All services are available to those with or without insurance. For those that are eligible, we offer a sliding fee scale. HealthCare Choices is a Patient Centered Medical Home that provides comprehensive, compassionate and continuous care for newborns, children, teens and adults. Our mission is to provide effective and reliable primary, multi-specialty and preventive health care in a personalized and compassionate manner to help people improve the quality of their lives.
Details: The School Health Program - Family Health Centers at NYU Langone provides medical, mental health, and dental services, along with health education, directly to children and adolescents through clinics located in public elementary, middle, and high schools throughout New York City. We care for children when they are sick, but also provide the preventive medical care to keep them healthy. By operating clinics inside schools, we help ensure that healthcare is accessible and convenient for children and families. A healthy child misses fewer days of school, which makes it easier to keep up with schoolwork and succeed academically. Also, when a child is healthy, his or her parents miss fewer days of work.

Rehabilitation medicine and physical therapy for adults and children are provided through Rusk Rehabilitation Brooklyn at Seventh Avenue Family Health Center. Dentistry and oral health care, HIV counseling and testing, and nutrition counseling are also available.
