Their commitment to advancing frontier healthcare and their approach to care has earned the trust of patients and their loved ones for 50 years. IFHS is a Patient Centered Medical Home (PCMH). The PCMH model is an approach to delivering high-quality, cost-effective primary care, and has been associated with effective chronic disease management, increased patient and provider satisfaction, cost savings, improved quality of care, and increased preventive care. IFHS uses a patient-centered, culturally appropriate, and team-based approach, coordinating patient care across the health system. We work as a team to help support maximum health and well-being for each of our patients.
