Details: In 2004, residents of the neighborhoods of Pilsen and Little Village on Chicago’s southwest side joined administrators from nearby Saint Anthony Hospital to address the critical lack of accessible health services in the area. They opened a community health center on South California Avenue called Esperanza, the Spanish word for “hope.” Today, Esperanza provides bilingual, high quality primary care, behavioral health and wellness services to the community, regardless of immigration status, insurance status, or ability to pay. Esperanza’s three sites in Little Village (South Lawndale) and Marquette Park (Chicago Lawn) deliver care to over 20,000 patients each year from communities across the Southwest side.
Details: Chicago Family Health Center (CFCH) Chicago Lawn Health proudly serves as a premier healthcare clinic and medical home for the residents of the southwest side of Chicago. We offer primary medical care for children and adults, as well as dental care, prenatal care and women’s health services, OB/GYN, behavioral health services, vaccines and flu shots, HIV testing, and STI testing and treatment. In addition to outstanding healthcare services, we offer support services for the whole family including care coordination and assistance with insurance and Medicaid benefits. Details: Friend Family Health Center, Inc. (“Friend Family”) was started with the development of the Friend Family Foundation by Berman and Hannah Friend in the 1930s. Nearly 70 years later, Friend Family applied for and was designated as a federally qualified health center (FQHC) to continue to ensure access to quality care in medically underserved areas. Since then, we have opened several service sites to deliver effective care to our communities, regardless of patient income. Today, Friend Family Health Center’s delivery sites provide care services to over 30,000 patients on an annual basis. We are proud to be recognized as a Patient-Centered Medical Home (PCMH) by the National Committee for Quality Assurance, a designation that demonstrates our commitment to improving the quality, effectiveness, and efficiency of healthcare delivery. Although Friend Family has grown significantly, both in number of service delivery sites and patients, we remain true to our founding philosophy: to provide more medically underserved and low-income individuals and families access to high quality health services while alleviating disparities in health care outcomes.

ACCESS provides a continuum of care model that connects patients to health care resources both within and beyond the walls of our 35 federally-qualified health centers (FQHCs). ACCESS' services are designed to address the health of our underserved communities in such areas as preventive care, chronic disease management, and support services. To address patients' comprehensive health needs, ACCESS physicians, nurse practitioners, midwives and other providers are teamed with outreach staff, case managers, social workers and substance abuse counselors to advance a continuum of care.
