About this clinic

Our main purpose as a Federally Qualified Health Center is to enhance the provision of primary care services in underserved urban and rural communities. Back in 1973, concerned citizens from Kerman, San Joaquin, and surrounding communities worked together to form an organization that could bring quality healthcare to this underserved region. The dream was realized when our first primary care center opened in the town of San Joaquin to meet the medical needs of the residents of the rural western Fresno County area. Valley Health Team has grown steadily to meet the mounting healthcare needs of the region. Our leadership is governed by an active and involved board of directors, some of whom are patients of Valley Health Team. Services are provided by a care team of qualified professionals working in multi-disciplinary teams. Board certified physicians, certified physician assistants, psychologists, nurse practitioners, licensed clinical social workers and health educators, dentists, optometrists, and support staff work with leadership to provide culturally relevant, outcome-based care. Valley Health Team is a Joint Commission accredited, private, non-profit Federally Qualified Health Center serving patients throughout the central California valley with an operating budget in excess of $48 million.

Is this clinic actually free?

What if I have no income at all?
Most sliding-scale programs have a lowest tier for people with no income, and many accept a signed self-declaration when you have no pay stubs to show. Ask for the clinic's discount program by name and ask what proof they take. Being told "bring proof of income" does not mean you are turned away for having none.
What should I bring to a first visit?
Usually a photo ID, something showing your address, proof of income or a self-declaration, your insurance or Medicaid card if you have one, and a written list of your current medications and doses. Call ahead — requirements vary, and arriving without the right paperwork is the most common reason a first visit gets rescheduled.
Do I need insurance, or immigration documents?
Federally funded health centers serve patients regardless of insurance status and do not require proof of citizenship to provide care. Volunteer and charitable clinics set their own rules. If it matters to you, ask before your visit what they require and what they record.
Is dental care included?
Usually not, and this is the single most common disappointment. Dental is frequently a separate program with its own eligibility, its own waiting list, and a narrower service list — extractions and urgent pain may be covered while fillings, crowns, root canals, and dentures are not. Confirm that dental is offered at this specific location before travelling.
Can I get a prescription refilled here?
Most clinics require an appointment before they will refill anything, and a first appointment can be weeks out. If you are close to running out of a medication you cannot safely stop, say so when you call — say the medication name and how many days you have left. Ask your pharmacy at the same time; pharmacists can sometimes authorise a short emergency supply.
What if I need care today?
