About this clinic

Knox-Winamac Community Health Center is a community-based organization that provides comprehensive primary care and preventive care services to persons of all ages, regardless of their ability to pay. The clinic was founded in 2006 by community leaders and in 2010 the clinic was designated as an FQHC-LAL (Federally Qualified Health Center, Look-alike). It is the Mission of Knox-Winamac Community Health Centers to provide quality, primary health care services, accessible to persons in our community including the medically underinsured and uninsured. Our Vision is to be the provider of choice for health care and health information for the communities we serve.
