Clinic Details:LCHC’s population consists of everyone on Lāna`i with a special focus on those who live below 200% of the federal poverty level-nearly 50% of the island’s population, or 1,500 individuals. No one is turned away because they lack money. LCHC serves individuals of all ages, ethnicity, gender, and residency - old timers, part-timers, and newcomers. LCHC accepts all insurances and assists its patients with understanding and navigating their insurance coverage questions. This health center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals. The Health Center is centrally located at 333 Sixth St. in Lanai City, where 95% of the population resides. Administration, Finance and Outreach are located at 333 Sixth Street, Lāna’i City, HI. LCHC is a 501(c)3 nonprofit organization, and a Federally Qualified Health Center (FQHC) with 330e status awarded in September 2007. Clinical services have been provided since August 2008.

Details: Founded in 1993 to meet the community demand for health services for the homeless, poor and underserved, Mālama I Ke Ola Health Center (MIKOHC) recently completed renovations on its new main facility and clinic at the old Ooka Supermarket. This modern, ‘green’ facility provides significantly more space for patient care and allows providers to deliver the full range of health care services needed by Maui’s rapidly growing community. Maui is an island with a land base of 727 square miles and a total population nearing 120,000. There is no public transportation system to link the many sparsely populated rural communities of the island to resources in central Maui.
Details: Molokai Ohana Health Care Inc. (MOHC), dba Molokai Community Health Center, is the only federally qualified health center (FQHC) on the island of Molokai, Hawaii. Our mission is to provide and promote accessible comprehensive individual and community health care to the people of Molokai with respect and aloha. The vision of the organization is that all people have optimal physical, mental and spiritual health. We plan to operate in a profitable, cost efficient manner that will expand quality and comprehensive medical, dental, behavioral health care, and social services to the people of Molokai regardless of their ability to pay. MOHC was created entirely by community volunteers and concerned residents of Molokai in response to the clear unmet need for expanded primary health care services on the island. MOHC has conducted an extensive community needs assessment, has garnered widespread community support, and is committed to primary care, medical, dental, and behavioral health on Molokai.

Details: Hui No Ke Ola Pono, Inc. is a private, not for profit community-based health enhancement, disease prevention and health care center. We provide programs on nutrition, health management and health care referrals for the community of Maui in a culturally caring manner. The staff of Hui No Ke Ola Pono works within a standard of conduct which reflects Hawaiian values. Hui No Ke Ola Pono is one of five Native Hawaiian Health Care Systems created under the Native Hawaiian Health Care Act of 1988 and reauthorized under the Native Hawaiian Health Care Improvement Act of 1992. Hui No Ke Ola Pono provides services to the Native Hawaiian Community on Maui through its Wailuku and Hana offices. The majority funding source is a federal grant from Health Resources and Services Administration (HRSA).
