Clinic Details:The UI Health Pilsen Family Health Center Lower West provides families and individuals of all ages with comprehensive healthcare, including immunizations, physical exams, prenatal care, and complex medical care. The 6,400-square-foot renovated facility features nine exam rooms and a laboratory for blood draws and basic testing.

Details: For more than 25 years, ACCESS has been on the frontlines of community-based health care. ACCESS provides a continuum of care model that connects patients to health care resources both within and beyond the walls of our 35 federally-qualified health centers (FQHCs). ACCESS' services are designed to address the health of our underserved communities in such areas as preventive care, chronic disease management, and support services. To address patients' comprehensive health needs, ACCESS physicians, nurse practitioners, midwives and other providers are teamed with outreach staff, case managers, social workers and substance abuse counselors to advance a continuum of care.
Details: Alivio Medical Center opened its doors to an eagerly awaiting community on January 4, 1989. On that day, the vision to create a community-based health center, responsive and culturally sensitive to many of Chicago’s most underserved and vulnerable communities, was finally realized. The Chicago neighborhoods of Pilsen, Little Village and Back of the Yards have been significant points of entry for immigrants and remain the home to a growing Mexican immigrant population in addition to an expanding community of Mexican Americans. Today, we serve families and individuals who historically have had limited access to healthcare for many reasons including: language and cultural barriers, intimidation of traditional healthcare institutions, limited awareness of available resources and/or fear of seeking help due to immigration status. Consequently, health promotion, education and prevention are integral parts of our comprehensive programs. Alivio’s bilingual, bicultural providers serve all patients regardless of ability to pay. Alivio Medical Center is a Federally Qualified Health Center and a grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n).
Details: This 5-story 121,750 square foot state of the art facility is Mile Square Health Center's primary practice site. This location on S. Wood St. at W. Roosevelt is right around the corner from UI Health's Hospital and its dozens of outpatient clinics, offering convenient referral services to all patients.

Details: In 2004, residents of the neighborhoods of Pilsen and Little Village on Chicago’s southwest side joined administrators from nearby Saint Anthony Hospital to address the critical lack of accessible health services in the area. They opened a community health center on South California Avenue called Esperanza, the Spanish word for “hope.” Today, Esperanza provides bilingual, high quality primary care, behavioral health and wellness services to the community, regardless of immigration status, insurance status, or ability to pay. Esperanza’s three sites in Little Village (South Lawndale) and Marquette Park (Chicago Lawn) deliver care to over 20,000 patients each year from communities across the Southwest side.
Details: The mission of the Bridgeport Free Clinic (BFC) is to provide medical care for the uninsured community of Bridgeport, where 35% of the population is Asian and, of the 22% uninsured in the community area, the largest proportion (28%) is of Asian descent.
Details: The Mile Square School Based Clinics are federally qualified school located health centers (FQHCs) managed in partnership with the University of Illinois Hospital and Health Sciences System’s Mile Square Health Centers. As FQHC's, our school health centers are committed to providing care to uninsured and under insured families. The Health and Wellness Centers are located in Chicago's public, charter and contract schools. All sites are staffed by University of Illinois Hospital and Health Sciences System health professionals who work collaboratively with school faculty and staff.
