Clinic Details:For more than 25 years, ACCESS has been on the frontlines of community-based health care. ACCESS provides a continuum of care model that connects patients to health care resources both within and beyond the walls of our 35 federally-qualified health centers (FQHCs). ACCESS' services are designed to address the health of our underserved communities in such areas as preventive care, chronic disease management, and support services. To address patients' comprehensive health needs, ACCESS physicians, nurse practitioners, midwives and other providers are teamed with outreach staff, case managers, social workers and substance abuse counselors to advance a continuum of care.

Details: The PCC Clinic at The Boulevard provides care only to individuals who reside at The Boulevard. The mission of The Boulevard is to provide high quality, cost-effective medical respite care, holistic support, and housing services to help ill and injured homeless adults break the cycle of homelessness, restore their health, and rebuild their lives. PCC providers offer comprehensive primary care on-site five days a week to residents.
Details: Erie Westside Health Center at Laura S. Ward Elementary School is a school-based health center that serves Laura S. Ward Elementary School students and their families. We provide primary care and health education to promote health, prevent disease and foster healthier lifestyles in the Southwest Humboldt Park community. The health center entrance is located on the west side of the building off Ridgeway.

Through our mission, LCHC is working to improve the health of the more than 50,000 patients who have chosen us as their medical home as well as improve the health of the community where we're located. For the 20-30% of our patients who have no insurance, LCHC is the place where health care services are still financially accessible. For community residents who are seeking jobs, LCHC is a place to secure employment. For students, LCHC is a place to receive training. For our peers, LCHC is a place to find ideas about providing healthcare for an underserved community. Details: Behavioral Health Services of Circle Family HealthCare Network provides services to adults and children who are diagnosed with serious mental illness, emotional disturbances, and/or have significant loss of normal functioning. Outpatient services are provided from our health center at 5002 W. Madison St. Residential care is provided for up to eight (8) CILA (Community Integrated Living Arrangement) adult clients through an off-site location as part of a special program.

Nurse Practitioners promote a comprehensive approach to health care and emphasize the overall health and wellness of their patients. MSHC Humboldt Park is fully equipped for those in the community with physical and mental health disabilities. We have state-of-the-art equipment that makes our clinic fully accessible for all people, including those who use wheelchairs or have physical limitations. Our staff also specializes in managing the healthcare needs of people living with chronic mental illness. The care you receive at MSHC Humboldt Park is directly connected to the entire UI Health System including a hospital, outpatient clinics, and specialty care.
Details: PCC provides care at PCC West Town Family Health Center in partnership with Norwegian American Hospital. This PCC site is located inside Norwegian American Hospital's professional office building.
