Details: Health Care to the Homeless provides health care via mobile van services to the homeless living along the Waianae Coast and Kalaeloa. A team of healthcare providers delivers services to the homeless. The overall goal of the program is to outreach via mobile health care services to unsheltered homeless individuals and families, reduce barriers to health care by making available case management services and other needed resources (screening, referrals to medical/mental health care, food, clothing, household items, employment, identification documents, etc.) to homeless individuals who might not otherwise receive assistance, with the ultimate goal of appropriate, affordable, and available housing placement and self-sufficiency.
