About this clinic

Since 1993, Southwest Louisiana Primary Health Care Center has been providing affordable outpatient medical and dental healthcare services to our community which includes St. Landry Parish, Northern Lafayette Parish, and Evangeline Parish. In addition, we will assist the citizens of St. Landry and surrounding parishes in developing their greatest potential and assisting them to attain a purposeful life through education, family support, and community resources. We are a non-profit corporation governed by a Board of Directors. Southwest Louisiana Primary Health Care Center is Joint Commission Accredited and PCMH (Patient Centered Medical Home) Certified through Joint Commission. We are also PCMH NCQA Level 3 Certified. We are an FTCA Deeming Facility. We also operate satellite sites at two local schools, Sunset Elementary (Sunset, LA) and North Central High School (Lebeau, LA).

Is this clinic actually free?

What if I have no income at all?
Most sliding-scale programs have a lowest tier for people with no income, and many accept a signed self-declaration when you have no pay stubs to show. Ask for the clinic's discount program by name and ask what proof they take. Being told "bring proof of income" does not mean you are turned away for having none.
What should I bring to a first visit?
Usually a photo ID, something showing your address, proof of income or a self-declaration, your insurance or Medicaid card if you have one, and a written list of your current medications and doses. Call ahead — requirements vary, and arriving without the right paperwork is the most common reason a first visit gets rescheduled.
Do I need insurance, or immigration documents?
Federally funded health centers serve patients regardless of insurance status and do not require proof of citizenship to provide care. Volunteer and charitable clinics set their own rules. If it matters to you, ask before your visit what they require and what they record.
Is dental care included?
Usually not, and this is the single most common disappointment. Dental is frequently a separate program with its own eligibility, its own waiting list, and a narrower service list — extractions and urgent pain may be covered while fillings, crowns, root canals, and dentures are not. Confirm that dental is offered at this specific location before travelling.
Can I get a prescription refilled here?
Most clinics require an appointment before they will refill anything, and a first appointment can be weeks out. If you are close to running out of a medication you cannot safely stop, say so when you call — say the medication name and how many days you have left. Ask your pharmacy at the same time; pharmacists can sometimes authorise a short emergency supply.
What if I need care today?
