Our clinic welcomes and treats all southeast Portland residents, regardless of their ability to pay, insurance, income, background and preferred language. Details: Student Health Centers are like having a doctor’s office at school. We offer comprehensive primary and mental health care services to all Multnomah County youth ages 5-18 at no cost. Even if you don’t attend school, you can still get care.
Details: There are times when stress, trauma, loss and other circumstances can lead to disharmony, depression, anxiety, confusion, family problems, chronic pain and even serious mental health challenges. NARA’s Adult Mental Health Program provides a variety of counseling and support services for adults ages eighteen (18) and older.
Details: Rosewood Family Health Center is located in the southeast section of Portland, and serves people from all walks of life. With 18 exam rooms, plus a minor procedure room and an on-site medical laboratory, Rosewood Family Health Center is designed to meet the medical needs of your entire family. We provide comprehensive family medicine with the health and wellness of all generations of your family as our goal. If you need women’s healthcare, school or sports physicals, immunizations or prenatal and post-delivery care, Rosewood treats all of you.
Details: Too often, patients who are ready for discharge are unable to leave the hospital because they are homeless and have nowhere to go. Central City Concern has partnered with the hospital system to create Recuperative Care (RC) so that these patients can be discharged to CCC housing. On-site staff are available to provide case management. Recuperative Care provides immediate housing, intensive case management and access to primary care at our Old Town Clinic. Patients are picked up from the hospital, given their own room and are immediately established with a primary care provider and case management team. Case managers check in daily with their clients, take them to follow up appointments and assist them with their daily needs. Additional addiction and mental health care services are available at the Old Town Clinic. Once clients are stable, they can focus on rebuilding their lives. Central City Concern specialists can help them get supportive housing, training, employment and the resources they need to recover and to become self sufficient. Recuperative Care staff works closely with Portland Area hospitals and CareOregon to identify patients in need of housing and case management.
Details: As those who have been through it can attest, recovery from addiction is a step-by-step process that requires tremendous determination and discipline. To fully recover, individuals must devote their energy to turning their lives around - physically, mentally and emotionally. When a person walks through the doors of the CCC Recovery Center, they have taken that first step. They begin by learning about the disease of addiction and the journey to recovery. They learn how to move beyond the moment of crisis, undergo treatment and achieve long term sobriety. CCC Recovery Center uses a multi-disciplinary approach and evidence based practices to provide the appropriate combination of medical care, mental health counseling, education, peer mentoring and supportive housing for each individual. The Recovery Center also offers a culturally-specific addiction treatment service to African American clients. With all the necessary resources under one roof, Recovery Center staff can work with different providers and agencies to provide a continuum of care. CCC certified Alcohol & Drug Counselors are highly trained and committed to successful treatment and outcomes. Many on our counseling staff are in recovery themselves, bringing a unique and profound level of understanding to every interaction. At the CCC Recovery Center, you’ll find staff who are passionate about helping clients in their recovery and in turn, individuals who are motivated to change their lives.
Details: Since 1968, Outside In has transformed thousands of lives by helping break the cycles of chronic homelessness, poverty, and poor health among Portland’s LGBTQIA+ community, people of color, those experiencing homelessness, and the underserved. Our health clinic and young adult programs strive to meet people where they are and provide safe, affirming spaces for our community to receive judgment-free care and support.
Details: Outpatient Addiction Treatment provides a range of integrated services and offers recovering persons hope and support. Clients may come to outpatient services from a residential program, from tribes throughout the United States, and as referred by other local partners to participate in services matched to their specific recovery needs. Services are offered throughout the day and in the evenings to allow greater access.
Details: Quality health care services are provided at both of our Integrated Health Clinics and are accessible through all of our programs. Health services are available from family medicine to women’s exams, diabetes testing to tobacco cessation. We serve our community from infants to elders at both the NARA Indian Health Clinic and the NARA Wellness Center.
