About this clinic

Located in Louisville's downtown, Park DuValle at City View originally opened and operated under the name The Community Health Center of West Louisville in 1986. Park DuValle at City View offers adult medical care for sick visits, check up and acute needs, as well as full sservice dental care. Crowns, fillings, bridges and more can all be treated by our staff. Women's health is also avaliable at the City View satellite location, birth control, pap smears and menopause evaluations are all offered. Any OB services can be referred to another physician. Pharmacy delivery is avaliable to the City View location. Any Rx from a PDCHC physician (any location) can be filled and delivered to the location nearest you. Park DuValle at City View is open Monday through Friday from 8am to 5 pm.
