To address patients' comprehensive health needs, ACCESS physicians, nurse practitioners, midwives and other providers are teamed with outreach staff, case managers, social workers and substance abuse counselors to advance a continuum of care.
Details: PCC Walk-In Wellness Center at West Suburban Medical Center offers urgent care for illnesses and injuries that are not true emergencies. Patients can be seen without a scheduled appointment for a variety of health conditions. In addition, any patient that does not have a primary care provider can follow up with a primary care provider at one of our health centers and make PCC their medical home.This is also the location of our Chemical Dependency Clinic, Ultrasound Clinic, and Sports Medicine Clinic.

Details: Alivio Medical Center opened its doors to an eagerly awaiting community on January 4, 1989. On that day, the vision to create a community-based health center, responsive and culturally sensitive to many of Chicago’s most underserved and vulnerable communities, was finally realized. The Chicago neighborhoods of Pilsen, Little Village and Back of the Yards have been significant points of entry for immigrants and remain the home to a growing Mexican immigrant population in addition to an expanding community of Mexican Americans. Today, we serve families and individuals who historically have had limited access to healthcare for many reasons including: language and cultural barriers, intimidation of traditional healthcare institutions, limited awareness of available resources and/or fear of seeking help due to immigration status. Alivio Medical Center is a Federally Qualified Health Center and a grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n).
