About this clinic

No health insurance and no preventive health care—two daunting challenges facing many Corona residents, complicated by high rates of diabetes, asthma, hypertension, obesity and tuberculosis. The neighborhood offered few alternatives where to go to receive treatment, other than a long wait at the local hospital emergency room. Enter Urban Health Plan. In 2009, we brought our expertise serving medically underserved communities to Corona and opened Plaza del Sol Family Health Center. Plaza del Sol is fully paperless – all patient information is kept on secure electronic medical records. Plaza del Sol provides the community with essential and cost-effective primary health care. Services including internal medicine, pediatrics, nutrition and radiology, are offered in one location, making it convenient and easy for residents to receive care. Combined with a convenient six-days-a-week schedule with flexible hours and a culturally sensitive, bilingual staff (80% of whom are from Queens) Plaza del Sol is just what the doctor ordered.

Is this clinic actually free?

What if I have no income at all?

What should I bring to a first visit?
Usually a photo ID, something showing your address, proof of income or a self-declaration, your insurance or Medicaid card if you have one, and a written list of your current medications and doses. Call ahead — requirements vary, and arriving without the right paperwork is the most common reason a first visit gets rescheduled.
Do I need insurance, or immigration documents?
Federally funded health centers serve patients regardless of insurance status and do not require proof of citizenship to provide care. Volunteer and charitable clinics set their own rules. If it matters to you, ask before your visit what they require and what they record.
Is dental care included?
Usually not, and this is the single most common disappointment. Dental is frequently a separate program with its own eligibility, its own waiting list, and a narrower service list — extractions and urgent pain may be covered while fillings, crowns, root canals, and dentures are not. Confirm that dental is offered at this specific location before travelling.
Can I get a prescription refilled here?
Most clinics require an appointment before they will refill anything, and a first appointment can be weeks out. If you are close to running out of a medication you cannot safely stop, say so when you call — say the medication name and how many days you have left. Ask your pharmacy at the same time; pharmacists can sometimes authorise a short emergency supply.
What if I need care today?
