About this clinic

Post Rock Family Medicine is composed of five separate practices: Daniel J. Sanchez, MD, Prairie Star Family Practice, LifeLine Family Medicine, Rooks County Health Center Clinic, and Solomon Valley Family Medicine. We have clinics in Plainville, Stockton and Palco. Our mission is to provide the people of Rooks County and northwest Kansas the opportunity to optimize their health through a patient-centered medical home that delivers professional, compassionate, excellent service. We all work together as a team to coordinate your care; when you see any of PRFM’s providers, that information is shared back to your primary provider through the electronic health record.
