Details: Our Albany Park community health center is our newest location. This health center will provide 4,000 new individual patients with primary care medical visits, behavioral health visits, and oral health screenings for children ages 0-4.
Details: PCC Salud Family Health Center moved into a newly renovated facility on April 8, 2013. The facility includes a large patient waiting and registration area, newly renovated exam rooms, offices and restrooms, as well as new windows, flooring, lighting, plumbing, roof, heating and cooling system, and building entrances with signage. As a result of the renovations, PCC Salud Family Health Center meets the requirements for Leadership in Energy and Environmental Design (LEED) silver certification from the U.S. Green Building Council.
