About this clinic

CCHC operates a Health Care for the Homeless program in which our care team travels to area homeless shelters to provide healthcare to the homeless population. The Health Care for the Homeless Program is a major source of care for homeless persons in the city of Springfield, serving patients that live on the street, in shelters, or in transitional housing. In 2015, CCHC served 1,112 persons experiencing homelessness. CCHC recognizes the complex needs of homeless persons and strives to provide a coordinated, comprehensive approach to health care including substance abuse and mental health services.
