Clinic Details:The San Antonio Neighborhood Health Center is located in the midst of the East Lake District, one of the most multi-cultural districts in Oakland. Residents from Mexico, Central America, Ethiopia, Eritrea, China, Hong Kong, Vietnam, and many other countries come to SANHC for comprehensive medical care services. Staff speak over 12 different languages in order to serve patients in their preferred language. SANHC expanded in 2007, almost doubling in size. The health center now serves approximately 12,000 patients including children, teens, young adults, pregnant women, older adults and patients with chronic illnesses such as diabetes and hypertension.

Details: Youth Heart Health Center is a new School Linked Health Center in the La Escuelita Education Center. All students that attend schools in the La Escuelita Education Center are welcome to use the center including students from La Escuelita Elementary School, MetWest High School, Dewey High School, and Yuk Yao Annex/Centro Infantil Annex Child Development Center. Siblings of students can access all services, as long as the siblings are between the ages of 3-21. Health insurance is not required and all services are provided without charge to students.
Details: Since 2000, the Roosevelt Health Center has been working to improve adolescent health and well-being. We provide access to comprehensive and high-quality health care services including primary medical and dental care, behavioral health services, and health education in a respectful & youth-centered environment. Services are free of cost to all students. Roosevelt Health Center is operated by La Clínica, in collaboration with Oakland Unified School District, Asian Community Mental Health Services, Alameda County Health Care Services Agency - School Health Services Coalition, and Elev8/Safe Passages Partnership.

Street Level Health Project is an Oakland-based community center dedicated to improving the wellbeing of underinsured, uninsured, and recently arrived immigrants in Alameda County.
Details: AHS provides a school-based clinic for Oakland High School students to meet the health needs of our young people. We reach more than 2,000 students per year-half of whom are Asian and Pacific Islander. The clinic provides confidential reproductive health services, family planning, and counseling.

Details: AMC is the oldest and largest of the Asian Health Services sites and serves over 16,000 patients with nearly 60,000 visits annually. Our team of doctors, nurse practitioners, physician assistants, nurses, health coaches, behavioral health specialists, and nutritionists provide high quality, comprehensive care in 9 Asian languages and English for patients of all ages.
Details: LifeLong TRUST Health Center recently opened, serving adults with disabling conditions and histories of homelessness who also participate in Alameda County’s General Assistance Program. The mission of the TRUST Health Center is to improve the lives of clients by providing high-value care that addresses clients’ physical, mental, and social well-being in one location. This collaboration between multiple government agencies and non-profit organizations will promote dignity, respect, and compassion in all interactions among staff and clients. Participating organizations will collaborate closely to support clients to find and maintain stable homes, to promote their overall health and well-being, and to develop or re-establish strong social support networks outside of the public services system.
