The overall goal of the Start Community Health Center is to improve access to quality and comprehensive health care services, improve the health status of our patients through prevention, early identification, and treatment of serious health issues. The health center’s service model will integrate preventative and primary medical care, dental care, mental health, and substance abuse treatment, case management and other enabling services. Based upon screening and requested needs, clinic staff will provide general preventative and primary medical care on-site. In order to provide all services, we will be working closely with other entities in the community to ensure a continuum of care approach.
