Details: CHRISTIAN COMMUNITY HEALTH CENTER is a community-based 501 (c) 3 not-for-profit organization founded in 1991 to address the lack of health care in the Roseland community on Chicago's southside. The agency was formed with a MISSION to provide high quality primary healthcare and related services to the community regardless of ability to pay, in a manner which demonstrates in word and deed the love of Jesus Christ. Since receiving Federally Qualified Health Center (FQHC) status in 2001, CCHC has propelled an expansion of programs and services. While primary medical care is our anchor program, CCHC has become equally renowned for our presence as a multi-pronged social service agency. By incorporating primary medical treatment into a comprehensive slate of supportive programs, CCHC strives to meet the holistic needs of our community residents. CCHC currently operates seven sites throughout the Chicago land area, including four community health centers, as well as a mobile health van providing a wide range of primary medical and social services.
Details: Chicago Family Health Center (CFHC) South Chicago is a state-of-the-art facility located in the heart of the city’s South Chicago neighborhood. We offer primary medical care for children and adults, as well as dental care, prenatal care and women’s health services, OB/GYN, behavioral health services, vaccines and flu shots, HIV testing, and STI testing and treatment. In addition to outstanding healthcare services, we offer support services for the whole family including care coordination, gentle yoga, the Women, Infants and Children (WIC) program, Healthy Start and assistance with insurance and Medicaid benefits. Details: Friend Family Health Center, Inc. (“Friend Family”) was started with the development of the Friend Family Foundation by Berman and Hannah Friend in the 1930s. Nearly 70 years later, Friend Family applied for and was designated as a federally qualified health center (FQHC) to continue to ensure access to quality care in medically underserved areas. Since then, we have opened several service sites to deliver effective care to our communities, regardless of patient income. Today, Friend Family Health Center’s delivery sites provide care services to over 30,000 patients on an annual basis. We are proud to be recognized as a Patient-Centered Medical Home (PCMH) by the National Committee for Quality Assurance, a designation that demonstrates our commitment to improving the quality, effectiveness, and efficiency of healthcare delivery. Although Friend Family has grown significantly, both in number of service delivery sites and patients, we remain true to our founding philosophy: to provide more medically underserved and low-income individuals and families access to high quality health services while alleviating disparities in health care outcomes.
