- Details: Conveniently located on the St. Bernard Parish Hospital campus along West Judge Perez Boulevard, sits the St. Bernard Community Health Center. This clinic in Chalmette offers patients a wide variety of services all housed in one location. Services at this site include: Adult and Pediatric Primary Care, Behavioral Health, WIC and even diabetes education classes. Unique to the St. Bernard location is the Industrial Medicine department. This department helps patients needing Department of Transportation physicals, Non-DOT physicals, Return to work confirmations, Workman’s comp, drug testing, Pulmonary function tests, Audiometric testing, as well as Respirator FIT tests.
