- Details: We are pleased to offer a variety of services at Community Care - Hanford, through more than six clinics, which are located on one campus. Services include: asthma education, cardiology, dental, dermatology, diabetes education, endocrinology, family practice, gastroenterology, general surgery, HIV education, internal medicine, nephrology, neurology, nutrition education, obstetrics/gynecology, ophthalmology, orthopedics, orthopedic spine, otolaryngology (Ear, Nose and Throat), pain management, pediatrics, plastic surgery, podiatry, psychiatry, psychology, urology, vascular surgery and CPHW-SPSP. A Teen Clinic for reproductive health is also open from 3:30 to 4:30 p.m. on Thursday at the Primary Care Services and Hanford Family Practice Residency locations. If you don’t have an appointment, you may visit our same-day appointment clinic that is available from 8 a.m. to 8 p.m., seven days a week. Adventist Health offers a free shuttle for Community Care patients who live in Hanford, Home Garden, Armona or Lemoore and need a ride to appointments in Hanford, Home Garden or Lemoore. Patients may request the shuttle service when scheduling their clinic appointment.

- Details: Family HealthCare Network (FHCN) is a nonprofit community-based organization that operates 19 federally qualified health centers (FQHC) throughout Tulare and Kings Counties. Our mission is 'to provide quality health care to everyone in the communities we serve.' The organization is governed by a volunteer Board of Directors and has developed a comprehensive, patient-centered health care delivery model with 23 total sites in Tulare, Kings, and Fresno Counties. Health center sites are located in Cutler-Orosi, Farmersville, Goshen, Hanford, Ivanhoe, Pixley, Porterville, Springville, Terra Bella, Three Rivers, Traver, Tulare, Woodlake, and Visalia. FHCN also operates the Ventanilla de Salud Resource Center located in the Fresno Mexican Consulate and a Community Resource Building in the community of Porterville.
