- Details: Do you know how to be homeless? Probably not, and you probably didn’t know how much bureaucracy is involved. At our drop in services at a Woman’s Place, we help women navigate frustratingly complex systems to receive the help they need. Our staff is skilled at dealing with both the system and the complex needs of our guests using trauma-informed support and case management. Community Forward SF runs a 56-person capacity at a Woman’s Place. We have only one requirement - identify as a woman. Once inside, our guests receive the tools for success they identify with a case manager. This can simply be a hot meal, vocational training, housing placement - we let our guests decide what success looks like. Regardless of what services a guest wants, a Woman's Place gives them safety from the traumas of the streets.

- Details: The Mabuhay Health Center (MHC) is a University of California, San Francisco (UCSF) student-run free community health clinic targeting underserved Filipino-American residents of San Francisco’s South of Market (SoMa) District. MHC started in 2009 through the efforts of founder Dr. Alvin Teodoro and a committee of community members, students from Bay Area universities, and health professional students from UCSF Schools of Medicine, Pharmacy, Dentistry and Nursing. Furthermore, MHC holds partnerships with the Bayanihan Community Center (BCC), Bessie Carmichael School/Filipino Education Center K-8 (BCE), and most recently, Canon Kip Senior Center. MHC Day was proclaimed on December 5th, 2009 by the City of San Francisco and is celebrated by the SoMa community each year. Every second Saturday of the month at Canon Kip Senior Center, MHC provides health coaching, health education to youth, health screening, history and physicals, medication therapy management, behavioral health services, and referrals to SoMa residents. The clinic also fosters an environment where preceptors, students, community members, and patients can teach and learn from each other about providing culturally sensitive, accessible, and high-quality health care. Previously, with the generous sponsorship of the Gilead Foundation, MHC has provided Hepatitis B and HIV screenings as well as Hepatitis B vaccinations for our patients. Currently, we have reached over 500 patients, been awarded Top 40 under 40 2010 Readers Choice Award by 7x7 Magazine, and established strong partnerships across San Francisco districts and in San Mateo County. In 2011, MHC further expanded its services to SoMa’s youth by collaborating with Bessie Carmichael School/Filipino Education Center K-8. Medical Respite reduces the number of emergency room visits its patients make, along with their contact with police and EMS personnel, thus saving substantial city costs, and helps connect its patients with longer term support services as they heal. Beds at Medical Respite cost less than 10% of hospital beds per night, which average $3,000 per night. The Sobering Center is a 12-bed facility that allows people experiencing the dual traumas of addiction and homelessness to receive the help and care they need to detox themselves. Working with DPH’s medically trained staff, we work with individuals - individuals with nowhere else to go to receive these services - to reach a place where they are safe enough to not harm themselves or others. The Sobering Center reduces the number of emergency room visits its patients make, along with their contact with police and EMS personnel, thus saving substantial city costs, and helps connect its patients with longer term support services as they heal. While we do not force sobriety, we offer the tools, counseling, and referrals should an individual wish to pursue a long-term solution.
