Details: Youth Heart Health Center is a new School Linked Health Center in the La Escuelita Education Center. All students that attend schools in the La Escuelita Education Center are welcome to use the center including students from La Escuelita Elementary School, MetWest High School, Dewey High School, and Yuk Yao Annex/Centro Infantil Annex Child Development Center. Siblings of students can access all services, as long as the siblings are between the ages of 3-21. Health insurance is not required and all services are provided without charge to students.
Details: LifeLong TRUST Health Center recently opened, serving adults with disabling conditions and histories of homelessness who also participate in Alameda County’s General Assistance Program. The mission of the TRUST Health Center is to improve the lives of clients by providing high-value care that addresses clients’ physical, mental, and social well-being in one location. This collaboration between multiple government agencies and non-profit organizations will promote dignity, respect, and compassion in all interactions among staff and clients. Participating organizations will collaborate closely to support clients to find and maintain stable homes, to promote their overall health and well-being, and to develop or re-establish strong social support networks outside of the public services system.
Details: Located on the east side of Lake Merritt, Frank Kiang Medical Center offers comprehensive health care services for patients of all ages. The center has 10 exam rooms and can serve more than 5,000 patients every year. The modern facility has unique features like larger exam rooms which accommodates our patients who often come to appointments with family members or interpreters. The center is named after Frank Kiang, a community leader and former chairman and CEO of Metropolitan Bank who passed away in 2007.
Details: The West Oakland Health Council is a non-profit Community Health Center providing primary care, mental health, and substance abuse recovery services to residents of West, North and East Oakland, Emeryville and Southwest Berkeley. Its mission is to improve the health and well-being of residents of these communities and to reduce morbidity, preventable disability and premature death.
Details: The San Antonio Neighborhood Health Center is located in the midst of the East Lake District, one of the most multi-cultural districts in Oakland. Residents from Mexico, Central America, Ethiopia, Eritrea, China, Hong Kong, Vietnam, and many other countries come to SANHC for comprehensive medical care services. Staff speak over 12 different languages in order to serve patients in their preferred language. SANHC expanded in 2007, almost doubling in size. The health center now serves approximately 12,000 patients including children, teens, young adults, pregnant women, older adults and patients with chronic illnesses such as diabetes and hypertension.
