At Valleywise Community Health Center - Avondale, you’ll find a variety of specialty and primary care services that will strengthen your mental and physical health and keep you and your loved ones feeling your best. Conveniently located just off of Van Buren St. and Dysart Rd., we’re ready to care for you via an in-person or telehealth visit. This location is also home to the First Episode Center, a place dedicated to making sure your adolescent and young adult loved ones are able to live happy lives and pursue their dreams - after or while experiencing psychosis.
