Founded in 1996, the Petaluma Health Center is a non-profit Federally Qualified Health Center (FQHC) , committed to providing high quality health care with access for all in Southern Sonoma County. PHC has built a strong reputation for it’s innovation and has been on the leading edge of providing comprehensive primary health care in a team based, patient centered system, emphasizing overall wellness and the value of the patient provider relationship. What should I bring to a first visit?

Do I need insurance, or immigration documents?
Federally funded health centers serve patients regardless of insurance status and do not require proof of citizenship to provide care. Volunteer and charitable clinics set their own rules. If it matters to you, ask before your visit what they require and what they record.
Is dental care included?
Usually not, and this is the single most common disappointment. Dental is frequently a separate program with its own eligibility, its own waiting list, and a narrower service list — extractions and urgent pain may be covered while fillings, crowns, root canals, and dentures are not. Confirm that dental is offered at this specific location before travelling.
Can I get a prescription refilled here?
Most clinics require an appointment before they will refill anything, and a first appointment can be weeks out. If you are close to running out of a medication you cannot safely stop, say so when you call — say the medication name and how many days you have left. Ask your pharmacy at the same time; pharmacists can sometimes authorise a short emergency supply.
What if I need care today?
