Clinic Details:Brockton Neighborhood Health Center (BNHC) is a non-profit, multicultural, community health center. Incorporated in 1992, BNHC began providing services in 1994 in a mobile medical van operating out of a church parking lot. In its efforts to meet the health care needs of the community, Brockton Neighborhood Health Center constructed a new $17 million community health center, and began seeing patients in this new facility in November 2007. The new facility doubled the health center’s capacity to serve low-income, diverse, medically underserved patients in Brockton and surrounding communities.

Think of Whittier as a one-stop shop where you can come for your primary care, wellness programs, eye and dental care, and behavioral health services. Urgent care is available whenever the Clinic is open and pediatric patients will always be seen - even without an appointment.

Details: South Cove Community Health Center ('South Cove') is the premier Asian community health center of Massachusetts. Founded in 1972, South Cove provides medical and behavioral health services and youth and family programs for approximately 32,000 clients at four locations in the greater Boston area. Our mission is to improve the health and well being of all medically underserved in Massachusetts, with a special focus on Asian Americans. This mission is accomplished by providing high quality, community-based health care and health promotion programs that are accessible and linguistically and culturally competent for these populations.

Details: For over 65 years, HCHC’s mission has been to provide accessible, affordable services to ensure the health of all Hilltown residents. We’re dedicated to the health and wellness of our Hilltown communities and provides a wide range of services at our five sites. In addition to medical and oral health/dental services, we provide behavioral/mental health services, substance abuse treatment, eye care, physical therapy, and laboratory services. We also provide numerous community services and supports. Federal funding supports some of this work, as do insurance payments, but these are not sufficient to cover all the services we provide. We rely on grants from foundations and other sources, as well as on individual donations, to support new initiatives and all of our community programs. We look forward to working with you as a team to ensure you receive the care and services needed to keep you and your family healthy.
