Details: The Mile Square Clinics are federally qualified school located health centers (FQHCs) managed in partnership with the University of Illinois Hospital and Health Sciences System’s Mile Square Health Centers. As FQHCs, our school health centers are committed to providing care to uninsured and under insured families. The Health and Wellness Centers are located in Chicago's public, charter and contract schools. All sites are staffed by University of Illinois Hospital and Health Sciences System health professionals.

Details: Friend Family Health Center, Inc. (“Friend Family”) was started with the development of the Friend Family Foundation by Berman and Hannah Friend in the 1930s. Nearly 70 years later, Friend Family applied for and was designated as a federally qualified health center (FQHC) to continue to ensure access to quality care in medically underserved areas. Since then, we have opened several service sites to deliver effective care to our communities, regardless of patient income. Today, Friend Family Health Center’s delivery sites provide care services to over 30,000 patients on an annual basis. We are proud to be recognized as a Patient-Centered Medical Home (PCMH) by the National Committee for Quality Assurance, a designation that demonstrates our commitment to improving the quality, effectiveness, and efficiency of healthcare delivery. Although Friend Family has grown significantly, both in number of service delivery sites and patients, we remain true to our founding philosophy: to provide more medically underserved and low-income individuals and families access to high quality health services while alleviating disparities in health care outcomes.

Details: Avicenna is a free clinic for people who are uninsured or underinsured. Avicenna is open on Sundays from 1-4pm. Avicenna is operated by volunteer team comprising of healthcare providers, healthcare professionals students and community members.

Details: The SIU Center for Family Medicine in Jacksonville, Illinois, was established in 2016 through the collaborative efforts of Passavant Area Hospital, the Morgan County Department of Public Health, and SIU School of Medicine. It is a family medicine practice providing comprehensive primary care services to infants, children, adults and the elderly. In addition it is housed in the Morgan County Department of Public Health facility so patients can also take advantage of the health department’s services such as their Immunization, Family Planning and Women, Infants and Children Programs. The health center accepts patients with or without insurance and we welcome our veterans.
Details: Decatur Family Medicine, located in Decatur, Illinois is a group of family physicians that are trained to care for the entire family. In addition, we are an accredited Family Medicine Residency which means we train physicians to be Family Medicine specialists with an emphasis on health care needs regardless of your age or gender.
