Harbor Care Health and Wellness Center welcomes you to your Patient Centered Medical Home. Caring about you is our most important job. Services are offered in all languages, including English and Spanish. Harbor Care Health and Wellness Center offers primary, preventive and behavioral health care services to children and adults who are homeless or at risk of homelessness, uninsured, under-insured, or low income. We also offer a sliding scale fee for self-pay patients, and accept Medicaid and Medicare, along with most insurances.
