Clinic Details:Erie Humboldt Park Health Center is our third largest health center and provides a wide variety of services for men, women and children - including adult primary care, women’s health care, behavioral care, pediatric care, prenatal care and outreach to enhance the health of the Humboldt Park community. It is also the home of Lending Hands for Life, one of the city’s oldest and largest comprehensive treatment programs for HIV/AIDS. You can come to our Humboldt Park health center for everything from free pregnancy tests and school physicals to cholesterol checks and diabetes management. We even offer counseling services to help you deal with family problems or emotional issues. Erie Humboldt Park also has a five-chair, state-of-the-art dental facility that provides comprehensive oral health services - from cleanings and fluoride treatments to crowns and root canals - to children and pregnant women. Adult dental patients also must be medical patients of Erie Family Health Center. It’s easy and free to get tested for HIV without an appointment at Erie Humboldt Park!

ACCESS provides a continuum of care model that connects patients to health care resources both within and beyond the walls of our 35 federally-qualified health centers (FQHCs). ACCESS' services are designed to address the health of our underserved communities in such areas as preventive care, chronic disease management, and support services. To address patients' comprehensive health needs, ACCESS physicians, nurse practitioners, midwives and other providers are teamed with outreach staff, case managers, social workers and substance abuse counselors to advance a continuum of care.

Details: Located at Division and Western, Erie Division Street Health Center lies in the heart of the West Town and Humboldt Park communities that we’ve been serving for more than 50 years. At Erie Division Street, we offer primary medical care for children, adults and older adults, chronic disease management, vaccines and flu shots, HIV testing, STI testing and treatment, prenatal care and women’s health services. This health center was formerly run by the Chicago Department of Public Health (CDPH). While Erie Division Street is now under our management, CDPH will continue to provide mammograms, family case management and the Women, Infants and Children program at the same location.
