Clinic Details:For more than 25 years, ACCESS has been on the frontlines of community-based health care. ACCESS provides a continuum of care model that connects patients to health care resources both within and beyond the walls of our 35 federally-qualified health centers (FQHCs). ACCESS' services are designed to address the health of our underserved communities in such areas as preventive care, chronic disease management, and support services. To address patients' comprehensive health needs, ACCESS physicians, nurse practitioners, midwives and other providers are teamed with outreach staff, case managers, social workers and substance abuse counselors to advance a continuum of care.

Details: This clinic, formally known as Integrated Health Care - West, will work together with you so it is as easy as possible for you to receive health and social services in your neighborhood, close to home. We provide holistic, integrated, compassionate physical and mental health services. We are your healthcare partners, helping you to manage and understand your health problems, and guiding you through the healthcare system. As an IHC site, what sets us apart from other Mile Square Health Center (MSHC) clinics is that we are fully staffed by outstanding Nurse Practitioners. Nurse Practitioners are registered nurses with advanced degrees, certification, and clinical training to provide health care services to individuals of all ages. Nurse Practitioners promote a comprehensive approach to health care and emphasize the overall health and wellness of their patients. MSHC Humboldt Park is fully equipped for those in the community with physical and mental health disabilities. We have state-of-the-art equipment that makes our clinic fully accessible for all people, including those who use wheelchairs or have physical limitations. Our staff also specializes in managing the healthcare needs of people living with chronic mental illness. The care you receive at MSHC Humboldt Park is directly connected to the entire UI Health System including a hospital, outpatient clinics, and specialty care.
Details: Erie Humboldt Park Health Center is our third largest health center and provides a wide variety of services for men, women and children - including adult primary care, women’s health care, behavioral care, pediatric care, prenatal care and outreach to enhance the health of the Humboldt Park community. It is also the home of Lending Hands for Life, one of the city’s oldest and largest comprehensive treatment programs for HIV/AIDS. You can come to our Humboldt Park health center for everything from free pregnancy tests and school physicals to cholesterol checks and diabetes management. We even offer counseling services to help you deal with family problems or emotional issues. Erie Humboldt Park also has a five-chair, state-of-the-art dental facility that provides comprehensive oral health services - from cleanings and fluoride treatments to crowns and root canals - to children and pregnant women. Adult dental patients also must be medical patients of Erie Family Health Center. It’s easy and free to get tested for HIV without an appointment at Erie Humboldt Park!
