Clinic Details:Near North Health Service Corporation (NNHSC), a 501(c)(3) non-profit Federally Qualified Health Center (FQHC), is one of the largest providers of community-based primary health care in Chicago. We provide medical services, social services, and nutrition education to medically indigent and uninsured individuals and families. Our philosophy reflects an approach that anticipates community needs and plans for expansions that are developed in accordance to these needs. Throughout our evolution, not only have our methods held up under the intense new demands of ever-changing communities, but also we have come through with shining results. Nurse Practitioners are registered nurses with advanced degrees, certification, and clinical training to provide health care services to individuals of all ages. Nurse Practitioners promote a comprehensive approach to health care and emphasize the overall health and wellness of their patients. MSHC Humboldt Park is fully equipped for those in the community with physical and mental health disabilities. We have state-of-the-art equipment that makes our clinic fully accessible for all people, including those who use wheelchairs or have physical limitations. Our staff also specializes in managing the healthcare needs of people living with chronic mental illness. The care you receive at MSHC Humboldt Park is directly connected to the entire UI Health System including a hospital, outpatient clinics, and specialty care.
Details: The PCC Clinic at The Boulevard provides care only to individuals who reside at The Boulevard. The mission of The Boulevard is to provide high quality, cost-effective medical respite care, holistic support, and housing services to help ill and injured homeless adults break the cycle of homelessness, restore their health, and rebuild their lives. PCC providers offer comprehensive primary care on-site five days a week to residents.
Details: PCC provides care at PCC West Town Family Health Center in partnership with Norwegian American Hospital. This PCC site is located inside Norwegian American Hospital's professional office building.

Details: Erie Westside Health Center at Laura S. Ward Elementary School is a school-based health center that serves Laura S. Ward Elementary School students and their families. We provide primary care and health education to promote health, prevent disease and foster healthier lifestyles in the Southwest Humboldt Park community. The health center entrance is located on the west side of the building off Ridgeway.
Details: For more than 25 years, ACCESS has been on the frontlines of community-based health care. ACCESS provides a continuum of care model that connects patients to health care resources both within and beyond the walls of our 35 federally-qualified health centers (FQHCs). ACCESS' services are designed to address the health of our underserved communities in such areas as preventive care, chronic disease management, and support services. To address patients' comprehensive health needs, ACCESS physicians, nurse practitioners, midwives and other providers are teamed with outreach staff, case managers, social workers and substance abuse counselors to advance a continuum of care.
Details: Erie Humboldt Park Health Center is our third largest health center and provides a wide variety of services for men, women and children - including adult primary care, women’s health care, behavioral care, pediatric care, prenatal care and outreach to enhance the health of the Humboldt Park community. It is also the home of Lending Hands for Life, one of the city’s oldest and largest comprehensive treatment programs for HIV/AIDS. You can come to our Humboldt Park health center for everything from free pregnancy tests and school physicals to cholesterol checks and diabetes management. We even offer counseling services to help you deal with family problems or emotional issues. Erie Humboldt Park also has a five-chair, state-of-the-art dental facility that provides comprehensive oral health services - from cleanings and fluoride treatments to crowns and root canals - to children and pregnant women. Adult dental patients also must be medical patients of Erie Family Health Center. It’s easy and free to get tested for HIV without an appointment at Erie Humboldt Park!
Details: Erie Teen Health Center is Chicagoland’s oldest and largest comprehensive health center for teens and young adults. Because we know that being a teenager isn’t always easy, we’re here to help you with a wide variety of issues - from low-cost contraception to pregnancy tests. Details: Located at Division and Western, Erie Division Street Health Center lies in the heart of the West Town and Humboldt Park communities that we’ve been serving for more than 50 years. At Erie Division Street, we offer primary medical care for children, adults and older adults, chronic disease management, vaccines and flu shots, HIV testing, STI testing and treatment, prenatal care and women’s health services. This health center was formerly run by the Chicago Department of Public Health (CDPH). While Erie Division Street is now under our management, CDPH will continue to provide mammograms, family case management and the Women, Infants and Children program at the same location.
