Clinic Details:This clinic, formally known as Integrated Health Care - West, will work together with you so it is as easy as possible for you to receive health and social services in your neighborhood, close to home. We provide holistic, integrated, compassionate physical and mental health services. As an IHC site, what sets us apart from other Mile Square Health Center (MSHC) clinics is that we are fully staffed by outstanding Nurse Practitioners. Nurse Practitioners are registered nurses with advanced degrees, certification, and clinical training to provide health care services to individuals of all ages. Nurse Practitioners promote a comprehensive approach to health care and emphasize the overall health and wellness of their patients. MSHC Humboldt Park is fully equipped for those in the community with physical and mental health disabilities. We have state-of-the-art equipment that makes our clinic fully accessible for all people, including those who use wheelchairs or have physical limitations. Our staff also specializes in managing the healthcare needs of people living with chronic mental illness. The care you receive at MSHC Humboldt Park is directly connected to the entire UI Health System including a hospital, outpatient clinics, and specialty care.

Details: For more than 25 years, ACCESS has been on the frontlines of community-based health care. ACCESS provides a continuum of care model that connects patients to health care resources both within and beyond the walls of our 35 federally-qualified health centers (FQHCs). It is also the home of Lending Hands for Life, one of the city’s oldest and largest comprehensive treatment programs for HIV/AIDS. You can come to our Humboldt Park health center for everything from free pregnancy tests and school physicals to cholesterol checks and diabetes management. We even offer counseling services to help you deal with family problems or emotional issues. Erie Humboldt Park also has a five-chair, state-of-the-art dental facility that provides comprehensive oral health services - from cleanings and fluoride treatments to crowns and root canals - to children and pregnant women. Adult dental patients also must be medical patients of Erie Family Health Center. It’s easy and free to get tested for HIV without an appointment at Erie Humboldt Park!
Details: Erie Westside Health Center at Laura S. Ward Elementary School is a school-based health center that serves Laura S. Ward Elementary School students and their families. We provide primary care and health education to promote health, prevent disease and foster healthier lifestyles in the Southwest Humboldt Park community. Details: Located at Division and Western, Erie Division Street Health Center lies in the heart of the West Town and Humboldt Park communities that we’ve been serving for more than 50 years. At Erie Division Street, we offer primary medical care for children, adults and older adults, chronic disease management, vaccines and flu shots, HIV testing, STI testing and treatment, prenatal care and women’s health services. This health center was formerly run by the Chicago Department of Public Health (CDPH). While Erie Division Street is now under our management, CDPH will continue to provide mammograms, family case management and the Women, Infants and Children program at the same location.
