The Center is open exclusively for Horizon House Program participants and staff members. All Horizon House Center patients have access to other health services such as Pediatrics, Prenatal Care, Gynecological, Dental, Social Work and Health Education at Fairmount Primary Care Center located at 1412 Fairmount Avenue by walking in or calling 215-235-9600 to schedule an appointment.
