About this clinic

Faith Community Health Center began operations in 1998 and joined Horizon Health Care Inc.’s clinics in 2016 through a merge with Prairie Community Health. The clinic is equipped with three exam rooms, one treatment room, laboratory facilities and digital x-ray technology. The Faith Community Clinic is staffed with two Physician’s Assistants, Alanna Chapman, PA-C, and Valarie Brown, PA-C.
