Details: Las Fuentes Clinic is a non-profit 501(c)3 corporation, with the goal of providing medical care to the uninsured people of Guadalupe, Arizona. Over 4,500 visits are made yearly to the clinic. Medical care consists of family medicine, women's health, acupuncture, chiropratic, naturopathic and traditional medicine. An extensive community outreach program provides health education to local community schools, agencies and events. In addition, Las Fuentes serves as a clinical training site for medical residents, medical students, nursing students and medical assistants.
Details: Our primary focus is serving women facing unplanned, untimely or unwanted pregnancy, or seeking abortion recovery servies. We provide free ultrasounds, free pregnancy testing and low cost STD testing. We also serve the significant others (men and families) of women who seek services in our clinic.

Our integrated model helps save our healthcare system nearly $90 Million each year. More importantly, we’re keeping 80,000 of our neighbors healthy. Located on the Tempe St. Luke’s campus, our current Tempe Clinic is available to serve our patients in the East Valley. Even though our Tempe clinic is one of our smaller locations, we still provide the same great service, including access to medication from our Baseline pharmacy. Our patients also have easy access to our WIC department, located just a floor below.

Our newest clinic opened in February, 2015 in response to a growing need from our community. The bright playground, designed with the help of local students, welcomes families into the clinic. The Gateway Clinic is located across the street from GateWay Community College and is easily accessible via bus or light rail.

Our Baseline Clinic sits at the foot of South Mountain and represents the early days of Mountain Park Health Center. Through renovations and expansions over the years, the Baseline Clinic now offers all Mountain Park services, including on-site dental and pharmacy. Renovations through 2015 will ensure we continue to meet the growing needs of this community.
Details: Opened in August 2011, the Balsz Health Center provides an integrative model of care with education, medical, dental and behavioral health. This health center is the result of a collaboration between HonorHealth and Educare Arizona. Educare is a national network of quality early childhood centers that combine the best of Head Start, child care, preschool and family support. The centers help at-risk children ages birth to 5 grow up eager to learn, as well as provide the skills needed to begin kindergarten. Based on the latest research and evidence-based strategies in child development, Educare and NOAH enables children and their families to have continuity of care. Each family is held by many hands within the integrated model, whether it involves education, medical, dental or behavioral health.
