About this clinic

At Heart City Health we take a multifaceted approach to healthcare that allows us to provide superior care, across multiple disciplines, to treat the whole person. Our services include family medicine, dentistry, behavioral health, and a full service pharmacy, all under one roof. We’ll even guide you through the insurance process so you can rest assured that you have the coverage you need. We employ licensed and certified professionals who are compassionate and provide high quality care. With support staff to guide you through your healthcare experience, Heart City Health ensures you feel supported, cared for, and at home. Established in 1993, Heart City Health Center is a 501(c)(3) non-profit Federally Qualified Health Center (FQHC) providing affordable, high quality medical, dental, and behavioral health care to residents of Elkhart County. This health center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233 (g)-(n). This health center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals. Heart City Health Center may take positions on or advocate with regard to public policy issues pertinent to its mission, consistent with the requirements of applicable Federal and state law. However, Heart City Health Center conducts all of its activities in an entirely nonpartisan manner. Heart City Health Center may acknowledge the elected officials who support us, but we do not endorse, support, or oppose candidates for public office, nor do we intervene in any election on behalf of or in opposition to any candidate of political party.

Is this clinic actually free?

What if I have no income at all?
Most sliding-scale programs have a lowest tier for people with no income, and many accept a signed self-declaration when you have no pay stubs to show. Ask for the clinic's discount program by name and ask what proof they take. Being told "bring proof of income" does not mean you are turned away for having none.
What should I bring to a first visit?
Usually a photo ID, something showing your address, proof of income or a self-declaration, your insurance or Medicaid card if you have one, and a written list of your current medications and doses. Call ahead — requirements vary, and arriving without the right paperwork is the most common reason a first visit gets rescheduled.
Do I need insurance, or immigration documents?
Federally funded health centers serve patients regardless of insurance status and do not require proof of citizenship to provide care. Volunteer and charitable clinics set their own rules. If it matters to you, ask before your visit what they require and what they record.
Is dental care included?
Usually not, and this is the single most common disappointment. Dental is frequently a separate program with its own eligibility, its own waiting list, and a narrower service list — extractions and urgent pain may be covered while fillings, crowns, root canals, and dentures are not. Confirm that dental is offered at this specific location before travelling.
Can I get a prescription refilled here?
Most clinics require an appointment before they will refill anything, and a first appointment can be weeks out. If you are close to running out of a medication you cannot safely stop, say so when you call — say the medication name and how many days you have left. Ask your pharmacy at the same time; pharmacists can sometimes authorise a short emergency supply.
What if I need care today?
