- Clinic Details: The Homeless Persons’ Health Project (HPHP) is one of three health clinics within the Health Services Agency of the County of Santa Cruz, and is a member of the National Health Care for the Homeless Council. Located at 115-A Coral Street in Santa Cruz, it is in the heart of community services, sharing a campus with the Homeless Services Center (HSC) and the River Street Shelter, part of Encompass Community Services. A member of the Homeless Action Partnership (HAP), HPHP staff work diligently with other HAP members towards solving healthcare and housing issues throughout Santa Cruz County. HPHP is a full service, primary care health center, serving the needs of the homeless and low-income populations of Santa Cruz County. Along with our full staff of medical professionals, we employ social workers, Mental Health Client Specialists, Integrated Behavioral Health counselors (therapists), benefits workers, and support staff, to meet the needs of our clients.
