Details: Health Access Network (HAN) is a Federally Qualified Health Center (FQHC) that was started in September 2003 and now has clinic locations in the rural communities of Lincoln, Medway, Millinocket, West Enfield, and Lee (Maine). HAN’s service area includes 19 organized towns, plus numerous unorganized plantations and territories, in northern central Maine with a total population of approximately 23,000 people.
Details: Community Clinical Services (CCS) Psychiatry is a Federally Qualified Health Center and a full-service behavioral health practice. Our practice is located at 100 Campus Avenue in Lewiston and some of our providers also provide care at our B Street Health Center and Renaissance School. We have a large staff of psychiatrists and psychiatric nurse practitioners and we provide psychiatric care and medication management to children, adults and elders. We have providers that specialize in children, providers that specialize in adults, and a specialized geriatric team for patients with memory issues. We are one of only three geriatric psychiatry programs in Maine! Many of our providers also work at the St. Mary’s Regional Medical Center’s in-patient behavioral health programs, so they are able to follow patients throughout a larger continuum of care, when needed. St. Mary’s Regional Medical Center also has Maine’s only Behavioral Health Emergency Room, and we often receive referrals for follow-up care. CCS Psychiatry is a specialty care practice and what that means is often the patient needs to be referred by a primary care provider first so that we can coordinate care with your provider and have up to date information on your current needs. Details: The Health Equity Alliance is working to expand our healthcare offerings, including a new service to provide limited free healthcare services – the HEAL Triage Clinic. The part-time clinic aims to reduce unnecessary emergency room visits and increase enrollment in primary care while meeting basic needs for clients, particularly those who are actively using drugs and the LGBTQ+ community. Housed within HEAL’s new Bangor offices at 304 Hancock Street, Suite 3B, the clinic is operated by staff and volunteer medical providers. HEAL offers basic healthcare focused on immediate, non-emergent needs. The HEAL Triage Clinic is designed as a low-barrier access point to meet basic needs; as such, it will not offer ongoing, comprehensive primary care. The clinic will coordinate with local federally qualified health centers, like Penobscot Community Health Center (PCHC), as well as hospital systems, to connect clients with longitudinal, comprehensive primary and dental care.
