Clinic Details:Ho`ola Lahui Hawai`i (HLH) was established in 1986 on the West Side of Kaua`i. Ho`ola Lahui Hawai`i is designated as the Native Hawaiian Health Care System for the county of Kaua`i by Papa Ola Lokahi in addition to being designated as a Federally Qualified Health Center by the U.S. Department of Health and Human Services.
Details: Health Care to the Homeless provides health care via mobile van services to the homeless living along the Waianae Coast and Kalaeloa. A team of healthcare providers delivers services to the homeless. The overall goal of the program is to outreach via mobile health care services to unsheltered homeless individuals and families, reduce barriers to health care by making available case management services and other needed resources (screening, referrals to medical/mental health care, food, clothing, household items, employment, identification documents, etc.) to homeless individuals who might not otherwise receive assistance, with the ultimate goal of appropriate, affordable, and available housing placement and self-sufficiency.
Details: Hui No Ke Ola Pono, Inc. is a private, not for profit community-based health enhancement, disease prevention and health care center. We provide programs on nutrition, health management and health care referrals for the community of Maui in a culturally caring manner. The staff of Hui No Ke Ola Pono works within a standard of conduct which reflects Hawaiian values. Hui No Ke Ola Pono is one of five Native Hawaiian Health Care Systems created under the Native Hawaiian Health Care Act of 1988 and reauthorized under the Native Hawaiian Health Care Improvement Act of 1992. Hui No Ke Ola Pono provides services to the Native Hawaiian Community on Maui through its Wailuku and Hana offices. The majority funding source is a federal grant from Health Resources and Services Administration (HRSA).

Details: Our Care-A-Van program provides mobile medical and social services directly to individuals and families in need across Oahu. Using medically-equipped vans and a mobile medical clinic, Care-A-Van’s outreach teams make scheduled visits to approximately 30 locations.
