Clinic Details:Since 1957, Mantachie Rural Health Care, Inc. has offered medical, dental and behavioral health services to residents in North Mississippi for over fifty years. MRHC was the first community health center in Mississippi to be recognized as a level 3 patient-centered medical home. As a recipient of Federal financial assistance, MRHC does not exclude, deny benefits to, or otherwise discriminate against any person on the basis of race, color, national origin, disability, age, or ability to pay in admission to, participation in, or receipt of the services and benefits under any of its programs and activities. This statement is in accordance with the provisions of Title VI of the Civil Rights Act of 1964, Section 504 of the Rehabilitation Act of 1973, the Age Discrimination Act of 1975, and the Regulations of the U.S. Department of Health and Human Services issued pursuant to these statutes at Title 45 Code of Federal Regulations Parts 80, 84, and 91. Mantachie Rural Health Care offers a reduced rate of pay program for patients with or without insurance based on income.

(CMCIA), d/b/a Jackson-Hinds Comprehensive Health Center was incorporated in August 1970 as a nonprofit 501c(3) organization to engage in planning and development of comprehensive health services for residents of poor, largely non- White, communities in Jackson, Mississippi and surrounding areas. Jackson Hinds Comprehensive Health Center provides a comprehensive continuum of health and social services to meet the needs of our clients. Jackson Hinds Comprehensive Health Center (JHCHC) is the largest community health center in central Mississippi. Since its inception, JHCHC has grown from a “one-bus-and-Sunday school-classroom” operation to become a multi-site, multi-disciplinary provider of health services in Hinds, Warren, and Copiah counties. Jackson- Hinds one of only two federally qualified health center (FQHC) in the state operating a clinic for the homeless and the only FQHC operating a HUD housing complex for low-income elderly and disabled residents.

Details: Claiborne County Family Health Center collaborates with the local schools in Claiborne County, by operating (2) school-based clinics providing primary medical and dental services. Services are provided by Board Certified Physicians and Nurse Practitioners.

Details: Delta Health Center, Inc. (DHC) is the first community health center of its kind. Also, it’s the first Federally Qualified Community Health Center (FQHC) in the United States. DHC is a non-profit organization that is located in historic Mound Bayou, Mississippi, where it opened its doors in 1965.
