About this clinic

The Davis County medical and dental clinics are located within the Davis County Health Department building in Clearfield, Utah, allowing patients to receive multiple services are one convenient site. We offer patients of all ages and backgrounds comprehensive primary healthcare services including well care, prenatal care, pediatrics, immunizations, women’s health, geriatrics, and treatment for acute illnesses andchronic illnesses such as diabetes, hypertension, and asthma. Full dental services are offered to patients of all ages at this location including extractions, fillings, root canals, partials, crowns and dentures. Services are provided by experienced, dedicated dentists and hygienists using state-of-the-art dental equipment in a comfortable, modern setting. This site also serves as the Utah Cancer Control Program for Davis County, providing services for cervical cancer screening and vouchers for mammography services. It is also Davis County’s only testing site for sexually transmitted infections.

<p>I am unable to reach your midtown office. The number directs me to the prompt to put in 411 if you do not have an extension. From here, the system does apparently not understand anything I say. I typed in numbers 1, 2 and 3-- trying each option as none of them were working. Nothing. I then called your Ogden office-trying to see if possibly I Could reach someone who could connect me to the right place, and the number they provide on their answering serice is not the same as the one listed on your website/business card that I recieveed upon my last visit.&nbsp;</p> <p>&nbsp;</p> <p>I am needing to make an appointmnet and don't know how.
