Details: Sonoma Valley Community Health Center (SVCHC) is a non-profit, federally qualified health center that opened in 1992 as a free-standing primary health care clinic. SVCHC began as a grassroots effort to address the lack of access and disparities of health care for the uninsured and underserved in Sonoma Valley. Since its inception, SVCHC has grown in capacity to serve Medi-cal, insured, underinsured, private insurance, private pay patients and provides sliding scale fees for those in need. In addition to medical services, SVCHC offers behavioral health, an in-house phlebotomy service, dental clinic, pharmacy services and certified bilingual enrollment professionals to help determine eligibility and enrollment options. We believe in providing access to quality healthcare to all of our neighbors in Sonoma Valley. Sonoma Valley is situated in the isolated far southeastern end of Sonoma County, California. It is designated as a Medically Underserved Populations (MUPs) and Health Professional Shortage Area (HPSA) due to its serious shortage of health care resources. The target population served by SVCHC includes people working in local agriculture, monolingual minorities, have low-income, are uninsured/under-insured (the at-risk and disenfranchised, including the homeless).
Details: Petaluma Health Center, Casa Grande Clinic provides primary care of adolescents attending Casa Grande High School. We’re located in a modular unit on the campus of Casa Grande High School. This clinic operates during the school year, from Jan-June and August-December.

Details: Since 2001, the Jewish Community Free Clinic (JCFC) has been providing completely free medical care to anyone in need, regardless of any discriminating factors. From our first days in a one room club-house to our fully equipped medical offices that we occupy today, the JCFC offers services to anyone in need who finds themselves without health coverage. In March of 2014 we received the donation of our own medical offices. This donation was made by a local family who understands the important role the Jewish Community Free Clinic plays in the community. This generous gift strengthens our long-term ability to ensure that no one in our community goes without affordable, quality, on-demand care. During the past year more than 2,500 adults and children came to receive services at the JCFC. Clients come from as far as 100 miles to be seen during one of our weekly drop-in clinics. Interpreters are always available for Spanish speakers.

Details: The Elsie Allen Campus is located on the Elsie Allen High School and serves teens ages 12-19 throughout Sonoma County. You don't have to be a student to use the health center. Santa Rosa Community Health Centers offers confidential, free services for all teens in the Santa Rosa area. The law in California allows teens to be treated for 'sensitive services' without notifying parents. This includes pregnancy tests and counseling, birth control, STI testing and treatment, HIV testing and counseling, and emergency contraception. This is a teen-only space, staffed by other teens, with no adults in the waiting room (the only adults are the health care providers).
