Park Place Family Medical Center - Norfolk Community Health Center

  • Norfolk VA, 23504
  • Contact Phone: (757) 393-6363
  • Clinic Details: In 1992, the Mayor for the city of Portsmouth established a Health Services Advisory Committee, representing all major Portsmouth healthcare organizations, conducted studies that revealed both a shortage and mis-distribution of primary care physicians able to provide primary care to an estimated 37,000 medically indigent Portsmouth residents. Many of these individuals were gainfully employed but did not have health insurance coverage. As a result, a large number of uninsured residents, as well as those insured by Medicaid and Medicare, but unable to find a doctor, resorted to the inappropriate use of the city health department and the hospital emergency rooms for their primary care. Hampton Roads Community Health Center, formerly known as Portsmouth Community Health Center, Inc., opened its doors as a federally qualified health center on May 14, 1995, in Portsmouth, Virginia, with two doctors, one family nurse practitioner and eleven clinical support and administrative staff.

    • Details: In its heyday, Fulton was known as the epicenter for African Americans in Richmond’s Far East End. Historic Fulton, as it is known today, had many African American owned grocery stores, department stores, a library, and many other amenities found in any small city suburb. It even had medical practices, although segregated. The community thrived for many years until the early 1970s brought floods from the James River which nearly wiped out Historic Fulton. In fact, nearly all the residents were given relocation funding to help them move away from Fulton. Following that exodus, many of the amenities found in Fulton also disappeared and the once vibrant Fulton community was demolished. Now, several years later, many of the Historic Fulton residents want to bring back the community their parents built and that many of them enjoyed as youth and adults. This included a community once again being anchored with a medical home for its residents. Hence, the Greater Fulton Community Medical Center was born out of this desire to bring back medical resources near where the current residents live, work, and play. (Greater Fulton Community is comprised of Historic Fulton, Fulton Hill, and the adjacent community of Montrose Heights.) Indeed, the Greater Fulton Community Medical Center has reinvigorated the vibrancy Historic Fulton and surrounding communities need to keep their bodies, minds, and souls healthy as they look to the future.

  • Orange County Free Clinic
    • Location: Orange, VA - 22960
    • Contact Phone: (540) 672-0793
    • Details: Our mission is to provide equal access to primary medical care and medication assistance to low income, uninsured and under-insured residents of Orange County, Virginia who do not have the resources to afford their basic health care needs. The Orange County Free Clinic opened 2006 as 501 C 3 Non-Profit organization to provide basic health care to qualifying adult individuals, those who are uninsured, underinsured and have incomes at or below 200% of the federal poverty level. The idea for starting a free clinic in Orange County was inspired by a group of committed physicians and community leaders sensitive to the lack of access to health care for low income, uninsured residents. Between July and December of 2006 there were 86 registered patients. To put it in perspective we have over 4,700 patients registered today. In 2007, the Orange County Health Department gave the Clinic borrowed space to operate on Tuesday nights for walk-in patients. Today, the Clinic operates in space located in the Silk Mill Complex. In-kind donations from Laboratory Corporation of America (LabCorp) and Culpeper Regional Hospital followed as did a partnership between Orange Pharmacy, Wilderness Pharmacy and Rappahannock – Rapidan Community Services Board. The relationship between many of these early community partners and the OCFC remains strong to this day. In 2009, the OCFC became a member of the Virginia Association of Free and Charitable Clinics. In 2011 the clinic became a member of the Orange County Chamber of Commerce. In 2016, The OCFC moved to a larger facility in the Town of Orange, Orange Virginia, 101 C Woodmark Street, providing easier accessibility, expanded hours, services and additional staff. This tremendous opportunity happened through the hard work and sponsorship of many generous and caring individuals!
