Hampton Roads Community Health Center, formerly known as Portsmouth Community Health Center, Inc., opened its doors as a federally qualified health center on May 14, 1995, in Portsmouth, Virginia, with two doctors, one family nurse practitioner and eleven clinical support and administrative staff.
