About this clinic

We are a free clinic that provides primary health care to uninsured and under-insured Polk County Residents. Medical services are provided by volunteer physicians and nurse practitioners. Limited diagnostic and laboratory services, medication, and diabetic testing supplies are provided when available. In addition wellness services are provided during clinic hours by appointment.
