About this clinic

The Care Clinic provides quality, community-based primary care to adults requiring specialized health care and social services. The PHMC Care Clinic opened in 1992 as a primary care and specialized health services clinic under the guidance of St. Joseph’s Hospital in North Philadelphia. In 2005, St. Joseph’s turned over the management of the clinic to Public Health Management Corporation (PHMC), then The Philadelphia Health Management Corporation, a nonprofit, public health organization that had more than 30 years of experience running direct service programs. PHMC Care Clinic is a Federally Qualified Health Center (FQHC) and participates in a Section 340B pharmacy purchasing agreement with Coordinated Care Network.
